Colorado: Lawn Lake dam failure flooded Estez Park, 1982 due to 80 year old incorrect installation of a gate valve that eroded the earth core of the dam. Madrid, Spain: golden obelisk that's supposed to shimmy: did it once when the King pressed the button but too expensive to run and maintain since, so shimmy it doesn't.
