        There's a story on the Daily WTF about how a manager modified their SMS sending system in production and put in a fake URL for people to click on.

        So he put www.xxx.com as the domain name until he had the proper one. Cue lots of angry parents complaining about how the company was sending them porn.

          I used to use examples of that in my Workflow training courses - that if you are going to loop through the hierarchy, you might want to have a limit somewhere.

          They did that mistake at an insurance company I was working with - kind of combined with Nick's example. Every 10 minutes that the server was down, send a notification to the DBA. If they don't reply within 10 minutes, escalate to their manager.

          Server crash on a bank holiday weekend - I think they were notifying God by the end of it.

                Met the rain as I drove home. It's coming down pretty steadily out there now

                Had to go for petrol and a few bits of shopping, which in theory ought to take about twenty minutes but, what with rain and rush hour, seems to have added fifty minutes to the journey
