Just to check, I deleted the instance and launched a new one.

It stopped working

OpsWorks is supposed to pass the necessary private deployment key to the instance, which can then use it to pull from GitHub. But it fails consistently. The only reason it worked previously was because I'd manually generated a new key on the instance then set that up as a read-only deployment key on GitHub, which meant the private key was still sitting in the instance's /root/.ssh folder.
