The point is, if most people don't wash their hands, you might as well not bother. The problem is that some muslims are thus refusing to abide by hygiene regulations, which will have knock-on effects for us all. Not all hospitals will have non-alcohol based hand lotions and I would not expect them to, so if they refuse to use the lotions it will help to propagate MRSA germs etc.
It's just the thin end of an unhygienic wedge.
