Pointless post with no context, but is it about keeping existing contracts open at renewal rather than having to go through the whole horrendous procurement cycle every time you get low on bandages and drip stands?

One would rather assume a competent procurement team are constantly monitoring for best value. Every private organisation I worked for was. I have seen the ops manager put a supplier over a barrel mid contract because he had other offers that were cheaper from reputable vendors.

Why not put a rider in contracts that all drugs will be cheaper to the NHS than buying from the top 10 high street sources?

You would think so, but it is absolutely not the case, they have to adhere to a set of rules that ensure you give all potential suppliers an equal share.

For one example, one lab I know of (from the MD of the company involved) was running a clinical trial that required some sophisticated diagnostic equipment. They were offered the loan of the necessary kit free of charge (other than the necessary reagents) but the Trus's procurement team refused to allow this, insisting the request goes out to open tender. Given the result of that decision would be (a) the purchase of a very expensive piece of specialised kit that would have no further use after the end of the trial and (b) the offering company's kit was more expensive than other, similar but less flexible options so would fail the tendering process, the trial was abandoned. His company weren't even allowed to offer a discount, the rules state that list price has to be the one used.

As a less extreme and rather more germane example, look up the cost of replacing a £3 fluorescent tube under PFI and H&S rules.
