I haven't seen why Israel is so far out. Was it literally that they paid a fortune ages ago to be top of the queue? Or that they produce their own? I know Israel has quite a large medical industry but not whether that means they, like India, have huge production infrastructure.

Good public health infrastructure according to this but if that is so France & Germany should be well ahead. I suspect its political will and organisation. There is at least partially the willingness to take a chance and order ahead while working with the vaccine creators.
How Israel Jumped Ahead on Vaccinations - The Atlantic
But the story of Israel’s success is arguably more about distribution than procurement. As Dany Bahar, an Israeli economist at the Brookings Institution in Washington, D.C., recently observed, focusing on what the Israeli government has managed to negotiate overlooks why it was in a strong negotiating position in the first place. It could present itself to pharmaceutical companies as an attractive “pilot country” for an effective mass-vaccination program for its 9 million–plus people because of its small size and the “vast public health infrastructure” that the state has invested heavily in over the past seven decades, building on a tradition of socialist-minded worker health-care cooperatives that preceded the state’s founding.Comment

UK ordered ahead too but Israel just seems an order of magnitude (or more) ahead so I wondered if there was some unique detail - like they own the factories and are getting first dibsOriginally posted by MaryPoppinsI'd still not breastfeed a naziOriginally posted by vetranUrine is quite nourishingComment

I'm sure I read somewhere that Israel will provide access to all their patient records related to the role out in exchange for being first in line for divvying up the vaccine. Lots of privacy issues raised but it beats being dead and the info provided will save lives across the world.Make Mercia Great Again!Comment

Israel seem to have a central management for their health service unlike our distributed ones. they also have conscription so the majority of their adults are militarily trained and can be recalled. Any leader will tell you organising a trained group and populace is far easier than dealing with the general public.Comment

The UK, for once in this pandemic, is doing a pretty good job.
In the USA I would expect that when the anti vaxers start being a problem the response will probably be "leave them to die, just keep them away from me!"Comment
