WHO DECIDES THREAT LEVELS
The Joint Terrorism Analysis Centre (JTAC) was created in 2003 as the UK’s centre for the
analysis and assessment of international terrorism. JTAC is responsible for setting international
terrorism threat levels and Ministers are informed of its decision.

Concerns are that there is an Islamic Muslim bomb maker on the UK mainland. The Muslim bombers themselves are usually sad lonely inadequate people, often with mental health issues, as you rightly point out. The skills (and mental ability) that are needed to successfully pull of an attack like this are not usually found in conjunction with someone prepared to blow themselves into tiny tiny bits. Hence the search for a team.
Question is, why wasn't he under closer surveillance, considering he'd just returned a few weeks ago from Libya.
