Really?? Survey Monkey is a reputable and credible company providing this service and has been used by many big names (I know I’ve done a couple of reviews using Survey Monkey).

Bearing in mind your posting history I'd ask you the question why do you want this? I can't see a client and it's staff being overly happy having to do this and I'd ask what the end game is for this feedback. Is this just an exercise to get lots of thanks and likes or is there something genuine behind it. I have never heard of any other contractor needing to do this so I'd question why you need to.
I'd also point out the issue with asking for feedback and the value of it when you get it. If you are selectively asking a few people who you know will give you good feedback then what's the point?
I genuinely think this is a bad idea unless you can come back with solid reasons why not.

I presume you mean career rather than 'carrier'? I just ask previous people I've worked with to leave a LinkedIn reference. They can do that as a personal thing rather than a formal review which a lot of places don't/won't do.
YMMV and this advice is worth what you paid for it.
