It's hardly surprising that your clients are asking serious questions about your ability to provide them with managed IT services when it's palpably obvious that you cannot even manage your own properly.
You do absolutely nothing well. The only things you don't fail completely at are the things you do a half arsed job of.
Getting the wrong 10 people in a room to take turns denying it's their responsibility is not 'effective decision making'. It's unfettered spacktardery. Increasing the frequency of the spasms will not help.
