        works fine until a tax funded (even partially) part of the hospital allows a private patient to jump the queue and a granny dies.

                Depends on the emergency.

                My mother had a shocking experience in A&E when she was critically ill with a perforated bowel with a junior doctor feeding her laxatives. It took him 6 hours to call a consultant who then had her in surgery immediately.

                  I know an ex Doctor ( struck off ) who used to get his pen light and wave it about a the patient's head in a Star Trek Dr McCoy manner making dzzghh dzzghh noises then would tell them that they were cured. Honest, that is a true story.

                    As luck would have it there is nothing wrong with her but I was simply amazed that this massive balls up could happen and no one seemed to care apart from our GP.
                    This seems to be a recurring theme in NHS problems: There is a chain of activity involving several players, but nobody (apart from the patient, who is ignorant of the process) has any stake in the success of the activity.

                    There is a failure to communicate between people.

                    It seems to me that patients need a professional follower-upper person involved in their treatment, who has the knowledge, authorization, and ability to communicate with all players in all locations, to keep tabs on progress and keep people accountable for their tasks and responsibilities.

                    Something like a cross between a project manager and a helpdesk system manager, perhaps?

                      They are called Macmillan nurses for cancer care and this service is supplied by a charity and they are superb
