Anyone contracted there ? How quickly can you get a visa ( B-1/B-2 ? ) after an offer from a client ? I assume most US agents demand on working through a US LTD?

You can't contract there.

B1/B2 visas are business visit visas not work permits. You need a H1-B which is more akin to the UK Tier 2 permit. As MF said, there is no US equiv. of the UK Tier 1 (formerly HSMP) permit.

...and if you could would you want to be taxable on worldwide income in the US even if you don't live there?
I consider not having a US visa or a green card a privilege.
I sometimes look into my personal documents and admire the fact that it isn't there.I'm alright JackComment

>>taxable on worldwide income in the US even if you don't live there?
there is an $85k exemption if you apply for it if you can prove you live overseas - there is a dual agreement with the UK so you dont get double taxed -
the UK also taxes you on worldwide income even if you don't live there - and I don't believe there are any exemptions .Comment

What about dual taxation treaties?"I can put any old tat in my sig, put quotes around it and attribute to someone of whom I've heard, to make it sound true."
