Update.
Spoke to the PM on this project at last clientco.
In the end they kept him at home for 8 weeks before bringing him back in. They dropped his rate 29% and he went back part-time. The project was cancelled completely and a new one raised in its place. The new project is now, no longer develop the solution itself but outsource it.
So looks like i played this well. The added bonus is, present clientco has the type of software and skills they need, so I'm going to pass all the details and contact information over to the sales team.

Having done a similar role before, it works when you have a good point person in the US. They attend meetings, provide feedback, direction, requirements etc.
If you have no-body or someone who is just plain useless, you get sidelined & you have no idea what's going on. Everything is run from the US Globally for clientco. It leaves these options
1) Goto the US Perm which is a non -starter
2) Increase Flights to the US & stay 2/3 weeks at a time - which is doable as long as they pay & don't do a travel ban (which also happened to me last time)
3) Do what you can and milk it until you cannot deliver any more.
4) Feck off.
