This is of course either a blessing or a curse with the app-based system, that it doesn't know who you are, which is precisely why the centralised version got so much resistance.
Great for privacy, but relies totally on people doing as asked. Like a lot of tech, in an ideal world it works wonderfully well but in a world with people, it doesn't. e.g. Alexa and all that. In idealistic sci-fi it's super cool that your house sees you fall over and scrambles the ambulance, but in practice it means you are under 24/7 surveillance.

They should only ask name/DOB/postcode and the system should tell them if the person in question was legally required to self-isolate, with police automatically sent if the person broken the law.
There is no place (and time) for “honour” based systems during deadly pandemic.

Mrs d had her flu jab Saturday and one question was "have you been asked to isolate or had a positive test?" Naively she said "is this just to cover you, surely nobody comes in in that case"... apparently you wouldn't believe (maybe you would) number of people turning up with covid for a flu jab.
