Encountered the same in civil service ....external accounts needed KeyPass - internal accounts only needed basic guesswork.
One supplier I dealt with used the same admin username and password across civil and commercial customers and still do. Worrying 😱

I did once have access to a treasury system that wasn't terribly well secured. It would have been quite possible to transfer a few billion to an offshore account and disappear.

Client passwords
At client, we have access to a corporate Pluralsight account, for online learning, the password for the account there is something like this:

On all the production systems, the passwords to petabytes of critical and highly sensitive data are usually something like this:

Password1
