It's like going into one of the many coffee shops where they have different names and different sizes for small, medium and large.

If you say "regular" depending on the shop you get small, medium or large so you need to spy the cup size first and point to it.

At MaccyD's regular = small; medium = bigger and large = a proper sized portion.
