He has been a bit down recently. He's had no work for a long time, and I've had to start working the streets again.

I'm 99% sure they get the latter only after the former has been extensively used, to establish they really are likely to be better off and understand what is involved. Nobody is in a rush to cut your balls off (well maybe yours specifically).
