Apparently cases are on the increase and despite the best efforts of the 'vitamin supplement' lobby who claim it's lack of vitamin 'D' it's actually due to feeding your Kids semi-skimmed and skimmed milk. Calcium uses the fat as the vector to get into growing bodies.
The official advice is babies should be on breast milk and young kids should be on full fat milk, and as this baby was breast fed according to your theory this case shouldn't have happened.
However in other European countries e.g. Austria they continued to do what the UK did until the 80s (though some places stopped it earlier) which is to tell parents to also give babies and young kids vitamin D as well.
The NHS has a habit of presuming there are only a few cases of a disease that the disease has eradicated so they can stop preventative programs. They forget that the drop to a few cases are due to their programs and vigilance - TB is another example of this.
