Dunno about a depressurised cabin at altitude - there may be a wheels up interlock as per other ac (747).
The 320 door has to be pulled in to the cabin in order to shorten slightly before being pushed out. The cabin pressure as you say has to be overcome - which is huge at altitude. It takes a few second on landing to equalise the pressure. The bits of the door that move in form a decompression protection device when they lock into the fuselage.
The max pressure difference from altitude alone could only be 1 atmosphere - so not significant. But at 500 mph it might be a lot.
500mph generates a big enough pressure difference to keep a huge plane with hundreds of people onboard in the air. So presumably the speed would also generate a very significant pressure difference over the door too.
