My guess:
Autopilot engaged
Fell asleep
Accidently nudged stick forward in sleep- autopilot disengage - only a short tone not a warning is produced. Low alt warning probably too late as pilots groggy.
Cabin crew not aware until too late due to steady descent and cockpit door is locked due to terror threat.But I discovered nothing else but depraved, excessive superstition. Pliny the youngerComment
