They don't just die though, the management of their chronic condition is expensive, last for years and usually takes them away from work so increasing the cost to society.
They get a year to sort it out then no prescriptions for drugs that treat the multitude of problems that they are elevating their risk of getting. Cut their welfare if they are out of work as well.

Plus VAT is charged on takeaways, alcoholic drinks, confectionery, crisps and savoury snacks (I copied this off the VAT guide).
