I dont put any company on it mainly as it would be:

    Chef
    Consultant
Me Co through Agency Co for Client Co

    M: 081 811 8181
    E: [email protected]

which just seems plain stupid, its only client emails so i simply use:

    Chef
    Consultant

    M: 081 811 8181
    E: [email protected]

But doesn't it look odd that your email ( & phone & fax) is blatently at a completely different company from the one under your name?

What's wrong with using just your name?
