    They might ask for a copy of the birth certificate, mat certificate doesn't seem to suffice.

    Fill in the employer details as best you can for when you were working. Add an explanation in the "Other Comments" section for the time out of work. They'll know you weren't in the nick...

    Or give the relevant authority a bell, they'll have a helpdesk for such questions.

    I wonder if someone could please help me with regards to completing my SC form.
