They might ask for a copy of the birth certificate, mat certificate doesn't seem to suffice.

Could ask your HR for guidance seems they are the ones asking you to fill it in?

Fill in the employer details as best you can for when you were working. Add an explanation in the "Other Comments" section for the time out of work. They'll know you weren't in the nick...Originally posted by Hoppy View PostHi
