I used SJD for company formation about 5 years ago and then used a different accountant.
It cost approx £110+VAT and was done within a day or so.
The amount of time you have spent looking around and posting you could have had it done and dusted by now.

Looks like Companies House have now set up a similar service of their own. None of it makes a lot of difference: a Ltd company is a Ltd company.
