      My thoughts exactly, filled it in and sent it back telling them to check what their company policy was.

      I've had some people come back with happy to give you a reference but company policy states no references given to contractors. The guy gave me references before but their HR peeps have said no as of last year. Still he did give me a reference on my linkedIn profile.
