          b) I'm only looking at project management roles.

          Yes, project management is a doddle. Until something goes wrong. It's the rescuing of projects that's challenging.

            Isn't the point that if the PM is doing their job properly, no-one should know when something goes wrong (mitigations etc).

            Like most theories, very elegant at first glance, difficult to manage in practice.

                Be wary of where that line of reasoning takes you.

                The "Lessons Learned Report" that every professional project manager wants to produce and disseminate is usually quashed by management fearful of people finding out things had gone wrong. So no lessons are learned. So the same things go wrong. Over and over and over...

                Part of good project management is ensuring that when things start looking as though they may go wrong, the causes are also considered so they can be dealt with (usually done outside of the project).

                Mitigation, by the way, is making excuses. A frequently abused term. You probably mean "prepared responses to risks". Sadly, the things that do go wrong are usually the ones NOT on the risk register, so there is no prepared response! (Well there is, but that's a different technique.)
