With the industrial revolution (and arguably before that with an emerging rich merchant class) came newly acquired wealth. To preserve status, the landed wealthy needed to develop codes of behaviour that alongside exclusive institutions could maintain their social status. Smoking does not necessarily cause lung cancer. Nor is lung cancer always caused by smoking. And when lung cancer is caused by smoking there are other causal factors.
