He had hair for the first few episodes, it's only once he starts to lose it because of chemo that he shaves it off, which I believe coincides with the first appearance of Heisenberg.

Oh, and even with hair he still looks a fair bit older which is to be expected considering Malcolm in the Middle must have been about 10 years ago.
