A few with post counts of under 5k have had custom titles.

All you have to do is think up a custom title (I got my tits out for cuk?), email admin and ask politely. Of course the last bit is the difficult part on cuk - a while round here on one ends up frothing at the mouth threatening to kill anyone who dares to ask you if you are well.
