A few years ago I got a call about a permie role and the she-pimp asked if I thought I was pricing myself to high. This was for less than a third of what I'm on now FFS. Maybe it's a woman thing.
I've been on the same rate, give or take 40 pounds a day for the last 4 years. It seems it's the going rate excluding expenses for my work, I'm happy with it and would not accept any less unless desperate which I don't see happening.

Proobably, yes, if it's a response to an ad, but I don't usually get work that way; I get work through a couple of agents who know me well and through a network of senior managers and programme managers who know what kind of person they want.
