It was never used in WWII: it was intended for use in the event of a German invasion, which (for the benefit of those who have been educated recently) didn't actually happen.
Then a bookseller found a copy of it a few years back and started flogging prints, and it sort of grew from there.

How did this happen? Who's to blame? Well certainly there are those more responsible than others, and they will be held accountable, but again truth be told, if you're looking for the guilty, you need only look into a mirror.
