My mrs sent her driver's licence in when we moved house and got one back with a completely different name on it - correct photo and address, wrong name.
I had the wrong date of birth printed on my passport a few years ago too. Simple typo by the passport office, but I had to send the passport back with my birth certificate to get it corrected as the typo was automatically assumed to be correct.
That sort of thing is one of the big reasons I am against ID cards - it will be chaos.

Rule Number 1 - Assuming that you have a valid contract in place always try to get your poo onto your timesheet, provided that the timesheet is valid for your current contract and covers the period of time that you are billing for.
