What's protocol if he does - would he remain in charge pending a new leader, would Raab take over, etc? Would an interim leader be able to perform cabinet appointments and would they actually be PM immediately?

Raab most likely as interim, yes, but it would be up to the party. He would notionally have the authority for all those things, and some appointments would be necessary for a functioning gov’t, but he would do the absolute minimum until a new leader emerged.
