Help with 'Personal Service Company Questionnaire'

I have received a raft of documents as part of my 'on boarding pack' for a new gig via a large agency. I will only provide services outside of ir35.

One of the requested documents is a 'Limited Company Compliance Questionnaire'.

It gives the following 3 options:

    A. PSC
    B. Umbrella
    C. Other (Companies operating a different structure should set out in full the manner in which they operate in Section C.).

I'm confused by some of the questions in A - (I want to stay opted in)- any suggestions please?

    (A) PERSONAL SERVICE COMPANY QUESTIONNAIRE

    3) Do you consider the companies for which you provide services to be clients or customers of a business carried on by the Company?
    Yes (go to Q4)
    No (go to Q5)

    4) Do you accept that the individuals you supply will be treated as outside the scope of the Agency Workers Regulations 2010?
    Yes or No

    5) Do you accept that the individuals you supply will be treated as inside the scope of the Agency Workers Regulations 2010 and will become eligible for treatment that is comparable to permanent employees of the Client?
    Yes/No

    Thanks ;-)

You don't want to opt in to the AWR, you want to opt in to the Conduct of Employment Agencies Act. They're different things.
