I read something once about how a company got complaints because their automated text service was sending porn to their users.

Turned out that a manager had set the dummy domain name to xxx.com in production because they thought it wouldn't be used by anything...

I was once nearly thrown out of a presentation in part of Glasgow because we'd abbreviated File Transfer Protocol on the slides, which didn't go down very well in that bit of the city...
