                Dunno how common a form is but the first time I got my current accountant to do the accounts he sent me a letter with weasely wording respecting IR35. So much so that ever since then the letter I enclose with the receipts etc always asks him to prepare the accounts on the basis that IR35 does not apply. Since then there's been no more weaseling but it does leave the responsibility firmly on my shoulders...

                  As a business owner your finacial affairs are always on your shoulders as you have to sign it off. It is also on your shoulders to make sure you advise your accountant correctly as well which is what I think the OP's accountant is checking. Accountants are providing you a service, not taking total responsibility of your affairs.

                    I sent my contract to my accountant and they determined my IR35 status.

                      Obviously you have given us no detail of this and I am sure some accountants can offer this service but from what you have put I would be very worried. It isn't your accounts job to dictate your IR35 particularly when they do not know the working conditions.

                      I would have this seconded by specialists at the very least.
