Program to create Table of Contents from PDF Bookmarks

Does anyone know of a program that will create a printable Table of Contents using the bookmarks already in the PDF?

There are some Adobe Acrobat add-ons such as TOCBuilder from Mapsoft which do this but you need Acrobat which is expensive.

Does anyone know of a free or low cost stand-alone program which will accomplish this?
