4) The stakeholders have just changed their mind rather than sign off. From the documented business scenarios (which they prepared in detail) and what is now being asked for it is evident this is what has happened.

1) your mate has written it down wrong and doesn't reflect what the stakeholder wanted, therefore a re-write is required and the stakeholders are correct in not signing things off.
or
2) your mate hasn't minuted and signed (or has an email audit) of any decisons made by the key stakeholder and has left themselves high and dry in a who said what finger pointing exercise (which they'll lose)
3) He's done 2 but can't manage his / her stakeholders (which include the project manager / programme manager).
So rather than blaming the world, their job is to rectify their own problem.

I actually broke the whole thing into a graphical 'jigsaw' puzzle using Clip Art so everyone could see how it fits together(Im dealing with non techies).
