That's exactly what I did for my wife last year, as well.

I made her a ham and tomato sandwich with mustard - it's her favourite, as well as mine!

Perri Lewis has some nice handmade things in her blog on the Guardian site today - meant for valentines day, but I'm sure they would work for birthdays too.
