This sounds like the Agency Worker Regulations which you cannot opt out from - if you are in business on your own account (apply the same rules as IR35) then you won't fall within the scope of the legislation

Just asks for Company Name, Reg No and whether I consider I'm in Business on my own account.

Agency Working Regs Query
Agency has sent a form to complete details 'to support compliance with the Agency Working Regs 2010."
Question is for them to assess whether assignment falls within scope of regs
States "Are you intending to provide your services through a Personal Services Limited Company or Umbrella.....la la la".
Just asks for Company Name, Reg No and whether I consider I'm in Business on my own account.
I'm limited.
