    Doesnt sound like SC, sounds like HR and Ltd Co checks.

    What have they told you to provide, and have you asked them what the checks are??

    No idea, and don't recall seeing that one in the many questionnaires I've filled out over the years (but it may be a new one of course). What's the context?
