No, I am here ...well you guess right it is for a perm role, but I do not think should make such great difference.
I worked both as perm and contract in the past and I can tell you that something contractors I worked with had less freedom than perms, for example for an IB where I worked as perm I saw contractors arranging holidays just when the management needed to keep them out of the office in order to balance the books; for many other things they I never saw such big difference in term of working style and flexibility.
The only big advantage of contracting is just ...much, much less tax.
About the hiring freeze I guess you right even though I received an offer anyway, so maybe few departments within the company have not been affected so far.

I don't think the OP is coming back, but as I mentioned earlier I have seen that exact template, so I'm pretty sure I know where it is for.
However it's for an "HR Interview" and AFAIK only for a permanent role. If evidence not provided then you'll fail the background screening (or not get that far).
I've not known of the HR Interview stage or that form be used for contract recruitment at the client.
What the OP probably doesn't want to know about (or perhaps why he hasn't come back) is the global hiring freeze that has just hit...

Sorry, but as a contractor, your contract should be commercially confidential.
I can't imagine they'd ask the same questions to Dell or HP about how much they charge another bank for servers and consultancy.
You're being treated as a disguised employee imho - they should simply be checking that you were engaged at the previous client from date x to date y as you had stated.

They want it to ensure that the clients you claim you had were actually paying you and therefore real clients.
I had this at current gig - refused at first and they said fine, they'd just fail the clearance. I'd actually inflated my previous rate to get an increase so was a bit nervous but I heard they just glance at it and check the relevant box.

My advice, if you want to accept the contract just fill it out truthfully, otherwise you will probably lose the contract. This is all part of their on-boarding process however annoying that may be.

"HR interview" sounds like this is a perm role, not exactly the right forum to ask around.
If by any chance it's for a contract - it's up to you to decide whether you are happy to provide the info or not. I have worked for 2 Tier1 UK banks and neither has asked for proof of previous income, the agencies might have asked me about my previous rate, to which I always reply with the advertised rate for the role in question.
