                None of them are, really. They are generally just pretty clueless!

                The system has been live for nearly two years now. And the DBA only spotted the size of the tables in August when there were 70 million records in the system (99.999% duff data). Now there are 316 million records in the system.

                There is so much crap data, that it's nearly impossible to fix the problem - I can write code to identify the duff data. But because it takes an hour to run, the database errors out because of the chance that the data has changed too much.

                Without kicking everyone out while they produce a fix, there's not much they can do. And they don't want to kick everyone out, because it's a world-wide system.
