well not exactly the same - obviously.

Kinda - I think last.fm is a community first and foremost, with streaming added on, whereas Spotify is just a streaming service. Well worth checking out though.

Absolutely. Although I'm having to steer clear of it at the moment as it's not the best thing to take if one has high blood pressure - which I do at the moment
