If they have multiple titles, then I guess they can choose which title they want to be known as - Camilla is always referred to as the Duchess of Cornwall, which is the lesser of her titles. Whether she (or the press) like it or not, she remains the Princess of Wales until Charles succeeds to the throne. Royals don't have a surname, so they take their fathers title as one (So William Wales, Henry Wales).
