To answer 9, yes he did, there were several points in the last episode that were filmed from the 1st person perspective, this linked with the fact that his brother in law commented in the previous episode that you probably wouldn't hear it if you were shot direct in the head. Plus, if you look closely a guy looks shifty at Toni before going to the toilet which is at a perfect right angle to his point of view. So, when the guy came out of the toilet he shot Toni without him seeing it and this happened straight after 'Meadow' his daughter walked in through the door, hence the reason he looked up and didn't react scared but smiled, he didn't see his killer coming..

Well I disagree!

Throughout the 6 seasons, we the viewer have been an onlooker, perhaps even a participant to the unfolding events. It's us (me) who gets shot in the last episode. My participation ends. My window into Tony's world is closed. Life for Tony carries on. We are just another victim/casualty along the way.
