Had a quick look at that health-on-line.co.uk. Looks like it's a pay first get refunded scheme. Is that correct?

No, it's 'proper' health insurance, you get to mix and match the cover to your specifications a little better than BUPA to tweak to premium, otherwise it's phone up a claimline, they authorise and you get treated, they pay the bills (within agreed medical price bands, same as BUPA)

I pay £61/m for me and my daughter, in and out patient cover, cancer cover, etc, etc
