There comes a time in your life when circumstance results in you contemplating your own mortality. There is a lot to be said for enjoying yourself on the journey if that means eating to excess or smoking (without making others 'enjoy it' with you) then why not. It is about quality of life, if the choice is 3 score and 10, healthy, able to do most of the things you want and then keel over stone dead OR 4 score with the last 10 with ill health, old folks home, sat in your own fluids with a mind turning to mush - then it's a bit of a no-brainer !
As mentioned also, we can't afford pensions for all these people anyway so the more that shorten their own lives the better ! Yes they will cost extra on the way as they ask medical science to correct a lifetime of neglect but some rational decisions around suitable procedures and medication, with the onus placed on people requesting expensive drugs and procedures to demonstrate lifestyle changes, weight targets, redirect Sky subscription into medical insurance etc should help.
