Changing company address

Hi Guys,
When i set up my company i gave the address as my current flat.

I have had to move flat and tried to get the bank to change company bank statements from coming to old address address, but they told me i need to inform companies house of change of address first.

I have found form 288c which is Change of particulars for director, is this the correct form?
