Web survey creation and hosting

I need to design a web survey for about 200-250 respondents. Key functionality has to include work demographic capture, approx 30 questions, including drop down lists and radio options, and easy data extraction. I'd strongly prefer to host this on my own website but, if not, it has to be somewhere not too ad-filled or with pop-ups.

Any recommendations for a survey creation or hosting tool?
